the HVMHC Crisis 'North Team' and serve both Goodhue and Wabasha

services. TITLE: On-Call Crisis Response Practitioner (overnight
hours) PROGRAM: Community Based Services JOB SUMMARY: To provide
crisis assessment, intervention and stabilization services for
mental health and substance use crises. JOB RESPONSIBILITIES AND
ESSENTIAL FUNCTIONS: Assume responsibility for being on-call
evenings and weekends as days scheduled. Respond to on-call crisis
calls and assess the situation for further follow-up. Provide
support and utilizeactive listening skills. Stay with individuals
in a crisis bed setting or in their homes. Develop plans with
individuals to assist them in restoring his/her prior functioning
level following a crisis. Attend monthly On-Call Crisis Team
meetings. Must be available to work irregular hours and be flexible
with scheduling. Complete the On-Call Crisis Training. Possess a
vehicle, valid driver license, and valid auto insurance. PHYSICAL
REQUIREMENTS FOR POSITION: Must be able to move in a manner
conducive to the execution of daily activities. While performing
the duties of this job, the employee must communicate with others
and exchange information. The employee regularly operates equipment
(listed below) on a daily basis. Occasional bending and lifting of
office materials may be required. NON-ESSENTIAL FUNCTIONS: Perform
other duties as assigned by the Program Director or Crisis Team
Coordinator EQUIPMENT USED: Telephone/cell phone, computer, office
equipment, and conference calling, vehicle. JOB QUALIFICATIONS AND
REQUIREMENTS: Must meet minimum qualifications as a Mental Health
Practitioner as outlined in MN Statute 245i.04: Sec. 245I.04 MN
Statute Experience: Must have experience with working with
individuals that have had significantly reduced levels of
functioning in primary activities of daily living. Worker Traits:
Must possess a high level of human relation skills and aptitude for
effective decision making. Must possess good planning,
organization, and communication skills (oral and written) as well
as problem solving skills. Must be able to handle difficult and
stressful situations in a competent manner. Must be able to
maintain confidentiality. Must possess a valid drivers license and
a willingness to travel as needed to client or organization
