Mayo Clinic seeks a compassionate and dedicated Social Worker to join our healthcare team, focusing on enhancing the well-being of our patients through expert integrated care. This role involves assessing patient needs, and designing and implementing strategies to meet those needs. This includes counseling, support in healthcare navigation, and coordination of community resources. The ideal candidate will be committed to fostering patient-centered care in line with Mayo Clinic's mission of delivering outstanding service and results. Conduct comprehensive assessments of patient social and emotional health. Develop and implement care plans to address patient needs in collaboration with healthcare professionals. Provide crisis intervention and support in cases of abuse, mental health emergencies, and other situations. Facilitate patient access to community healthcare resources. Advocate for patient rights and services across healthcare settings. Document patient interactions and interventions accurately for continuity of care. Strong interpersonal and communication skills Expertise in behavioral health interventions Ability to navigate complex healthcare systems Crisis intervention skills Empathy and professional resilience Medical Insurance Dental Insurance Vision Insurance 401k Licensed Clinical Social Worker (LCSW) preferred Certification in mental health practice (e.g., C-SWHC) is a plus
