Conduct initial assessments and provide ongoing mental health counseling and support. Develop and implement personalized care plans in response to patient needs. Work collaboratively with healthcare teams to coordinate patient care. Advise on community resources and ensure clients make beneficial connections. Maintain up-to-date records of patient sessions and outcomes. Utilize evidence-based practices to facilitate patient mental health and wellness. Provide crisis intervention services as necessary. Strong clinical and ethical understanding of mental health practices. Ability to utilize a variety of therapeutic models and frameworks. Excellent interpersonal, communication, and case management skills. Proficient in maintaining comprehensive and accurate patient records. Adept in crisis management situations with calm judgment.
