Position Summary: This is a field-based role for a Certified Nurse Practitioner (CNP or AGNP) interested in wound care and long-term primary care. No certification in wound care? No problem. We’ll help you get there—and cover the cost. Key Responsibilities: Visit multiple senior care sites each week Deliver in-place wound evaluations and follow-up care Participate in care coordination and medication reconciliation Build trusted relationships with residents, families, and facility teams Qualifications: Current MN CNP license Wound care certification or willingness to obtain Background in long-term care or family/geriatric practice Must have reliable transportation Military Occupational Classification (MOC) Codes: Army: 66P Navy: 1972 Air Force: 46Y3H Marines: NP Coast Guard: HS Join Our Team: You became an NP to make a difference.
